The Consular Office notarizes, free of charge, original supporting documents issued by Swiss hospitals (i.e. documents carrying the original hospital stamp or the signature by the responsible department). In case of presentation of the supporting documents by a third party, a certified authorization is required from the beneficiary insured to the person appearing at the Consulate. The latter must present proof of identity that bears his / her photo, eg Passport or Greek ID.

If you wish to send the documents by post, they must be accompanied by a prepaid registered envelope for the return of the documents to you. The Consular Office bears no responsibility for any loss of documents during their dispatch.
