Vital Records

For birth and marriage registrations, the physical presence of the interested parties before the Consular Authority is required. For death registrations, the physical presence of the declaring relative, funeral director, or any other person with a legitimate interest is required.

General requirements regarding the supporting documents required by the Consular Authority to draw up a registration document are the verification of the identity and the documentation of the applicant's will, as well as the verification of the identity of the person to whom the registration document is drawn up, and are specified as follows:


1. Birth Certificate:

i.  Parents' IDs/Passports

ii. Detailed Birth Certificate (Odpis Zupełny)


2. Marriage Certificate:

iii. Spouses' IDs/Passports

iv. Detailed Marriage Registry Act (Odpis Zupełny)


3. Death Certificate:

i.   ID cards/passports of the deceased and the declarant.

ii.  Detailed Death Certificate (Odpis Zupełny).

iii. Medical Death Certificate .

iv. Document with funeral or cremation details.
