Declaration of Death

For the declaration of death a relative of the deceased or a third party (e.g. an employee of an authorized Funeral Service) must attend in person.

Required documents:

1.    Passport and identity card (if any) of the deceased person 
2.    Official medical certificate from a hospital stating the exact cause of death 
3.    Original Death certificate issued by the competent Slovak authorities accompanied by a multilingual translation.
4.    Certificate of family status of the deceased (ex officio search)
